All employees are encouraged to get a flu vaccine annually. Willamette University has partnered with Kroger Health to offer flu shots at our upcoming clinic. This year, we will hold flu shot clinics for faculty and staff on both campuses. Additionally, we will have a limited number of flu shots available for dependent children and spouses. To be eligible, employees and their family members must be 65 years old or younger. Please note that Kaiser Insurance members must be 18 years or older to participate.

Locations:
Salem Campus - Wednesday, October 2, 10am - 2pm in Henkle Gym West (lower level of Sparks). (Sign ups close on Monday, 9/30, at 4:00pm)
Due to low sign-ups, the Flu Shot Clinic at the Portland Campus has been cancelled.
An appointment will be required this year (walk-ins will not be allowed).
Please schedule your flu shot appointment through WISE and click on the tab labeled "Sign-up," on the left side of the page.
To Receive A Shot, You Must:
- Complete the Vaccine Consent Form and email it to tdegroat@willamette.edu. *You do not need to fill in your social security number or your insurance member number;*
- Make a copy of your insurance card to your vaccination appointment. If you cannot make a copy of your insurance card, please email tdegroat@willamette.edu;
- Be age 65 or younger;
- Be age 18 – 65 if you are a Kaiser Insurance member (if not insured by Kaiser, the clinic can provide shots to those as young as 9 years of age);
- Not had an anaphylactic-type reaction to latex;
- Not have an allergy to chicken eggs and/or egg products;
- Not have an allergy to Thimerosal (used as a preservative in vaccines);
- Not be exhibiting symptoms other than mild coughing, runny nose, and/or diarrhea;
- Not have a history of Guillain-Barré Syndrome;
- Not had a serious reaction after receiving a prior influenza and/or pneumonia vaccination;
- Meet any CDC eligibility requirements required at the date of service; and
- Ensure you are dressed appropriately so the nurse can reach your upper arm.
*Clinic providers encourage you to have a snack and come hydrated.
