A variety of online report forms have been created for easy referral and follow-up for student or campus concerns. These forms can be found on the Student Affairs website along with additional university support resources.
CARE report
The CARE report is a way for members of the community to ensure a student who is experiencing difficulty is offered support and resources. CARE stands for Campus Assessment, Response and Evaluation. Submit a CARE report when you have:
- General well-being concerns for a student, indicated by changes in behavior or personal upkeep, substance use, concerning comments or missing commitments.
- Academic concerns such as test preparation, study skills, writing support or concern for a student's academic performance.
Note: CARE reports are not for immediate emergency concerns. For emergencies, contact Campus Safety at 503-370-6911.
The confidential form serves as a request for follow-up by an appropriate staff or faculty member. It provides written notification to the CARE team.
This team, led by the Director of Bishop Wellness Center Don Thomson and Associate Dean of Student Success Gretchen Moon, meets weekly with member representatives from student support offices to discuss the CARE reports and coordinate support and follow-up with the student of concern.

Bias report
Willamette University does not tolerate discrimination in its activities or employment opportunities on the basis of race, color, religion, sex, national origin, marital status, veteran status, actual or perceived sexual orientation, gender identity, gender expression, or status with regard to pregnancy, disability or age. We are firmly committed to responding to incidents of bias which can include use of slurs, epithets, symbols, graffiti, degrading language, harassment or exclusionary practices. Sexual misconduct report
The university is committed to fostering a fair and equitable environment for individuals to study, learn and work and, therefore, prohibits sexual and gender-based harassment, sexual and gender-based stalking, interpersonal violence (including dating and domestic partner violence), sexual exploitation, non-consensual sexual contact, non-consensual sexual intercourse, and retaliation against a person for the good faith reporting of any of these forms of conduct. For gender-based harassment, sexual violence, stalking and interpersonal violence submit a sexual misconduct form.
Reports of potential violations of this policy will be kept private to the extent possible for the university to respond to the report, but reports made to those listed below are not confidential.

- Sexual Assault Response Allies (SARAs) — SARAs are specially-trained Willamette University student volunteers who provide confidential support and resources, 503-851-4245.
Employees may seek out confidential resources through the Employee Assistance Program or other local health and counseling resources.
Community concerns/incidents
Concerns about campus issues, reports of a campus incident that needs follow up or concerning behavior that is affecting you or other members of the community can be submitted on a community concerns/incident report. By communicating with our community about what kind of behavior we expect from each other, we strengthen our community standards. Reports are referred to the appropriate university personnel to address the concern.
COVID-19 self reports
We ask all members of the Willamette community to self-report to the university if they receive a positive COVID-19 result to ensure that we can apply appropriate safety measures and support. Students should self-report their positive COVID-19 status to Tori Ruiz, student CARE and conduct manager, Truiz@willamette.edu, 503-370-6447, or by submitting a COVID-19 CARE report form. Faculty and staff should report their positive case status to Human Resources at 503-370-6210 or by submitting this Employee COVID-19 report form.
